WondrPath
A confidential health snapshot for your community.
A short, confidential health survey that tells you where your health stands and what to ask your doctor about. Community organizations can also run it to understand the health needs of the people they serve.
Take the Survey
WondrPath is a short, confidential health survey. Anyone can take it, and it's free.
When you finish, you get a detailed personal health report: where you stand across the areas the survey covers, which screenings and check-ups are generally recommended for someone of your age and risk profile, the published guidance each recommendation is based on and why it applies, and health resources available in your area.
The report is written to be something you bring to a doctor. It doesn't diagnose anything and it isn't a substitute for medical advice, it's meant to help you have a better-informed conversation with a clinician about what to do next.
Nobody is asked for their name. You can optionally give an email address or phone number at the end if you would like a copy of your report sent to you.
How It Works
- 1
Answer the questions.
It takes a few minutes and you go at your own pace.
- 2
Read your report.
You get it immediately, with recommendations, the guidance behind them, and local resources.
- 3
Take it to a doctor.
The report is designed to be shown to a clinician who can advise on next steps.
For Congregations, Schools, and Community Organizations
Organizations can run WondrPath under their own name and branding. Everyone in your community takes the same survey and gets the same personal report, and you get a picture of where your community's health needs are greatest.
Results come back as charts and summary analytics across the whole group. No individual is ever identified, and each partner sees only their own community's results.
Several partners have used what they found to apply for funding and build programs that respond to real, measured need rather than assumption.
We currently partner with African American congregations, communities where health disparities are well documented and where locally led programs are often the ones that work. We're expanding the pilot.
Privacy
Participation is confidential. No one is asked for their name. Participants may optionally provide an email address or phone number at the end if they want a personal summary sent to them, and that contact information is used only for that purpose.
Partner organizations receive charts and summary analytics for their own community only, never another organization's, and never individual responses. The Foundation does not sell or trade any information collected through WondrPath.
Full details are in the consent form every participant reads before taking part, and in our Privacy Policy.
WondrPath is part of the Foundation's community health work, alongside WondrVoices.
